Take a look at a new video campaign featuring local schoolchildren aimed at reminding people across the North East that a course of antibiotics is not required to treat common winter illnesses such as viral coughs, colds and flu.

The following link has information about self care for coughs, sore throats, ear ache and sinusitis.

New Patient Registration

handwriting_from_screenTo register with the Practice, please visit the Front Reception desk in person to obtain the necessary paperwork for completion.

Alternatively, you can print off forms from the links below in advance and bring them in to us.  You will need to complete 2 forms for each person being registered: 1) Registration Form (GPR) and 2) New Patient Questionnaire - there are different forms for adults and children.  When you have completed both forms, please bring these to the surgery, along with two forms of ID - one photo ID and the other a document showing your name and home address, eg utility bill.

If you have a preferred Doctor, ie by name or gender, we will note this and do our best to respect your choice. However not all the Doctors in the Practice provide all services and specific Doctors may not be immediately available.
