CORONAVIRUS

If you have a new cough or high temperature you must self-isolate.  Follow self-isolation advice on NHS Inform www.nhsinform.scot or phone 0800 028 2816

14 day self-isolation certificates for employers can be found on NHS Inform:  https://111.nhs.uk/isolation-note

If you have a cough or fever and you need medical attention telephone dial 111 for NHS 24.  Patients in high risk groups should phone 111 as soon as they get symptoms.

Essential Care only - Do not attend your GP practice in person - phone from home

PLEASE NOTE THAT OUR DOORS ARE LOCKED.  Access will be for pre-arranged appointments only for essential care.  Please bring your mobile phone with you.

On arrival at front door, a notice will advise you to call a mobile telephone number to let us know you have arrived.  You will be asked to wait in your car.  Tell us if you arrive on foot/by bus.  We will call you back to tell you when to enter the building.  If you do not have a mobile phone - you will need to knock on the window to alert our Receptionist.  If she is away from the front desk, please be patient.

Adults to enter alone, unless carer required.  Children under age 16 may be accompanied by one carer.

Note:  Our most vulnerable patients will be sent a letter directly from Scottish Government.  Patients in this group are advised to follow shielding advice - click here to view advice.  A copy of the letter being sent to patients can be found at the link below:-

Repeat prescriptions - our timescale for preparing prescriptions has been extended to 4 working days. All prescriptions will be sent to a nominated pharmacyAllow a further 2 working days before attending the pharmacy.  If you have not nominated a pharmacy your prescription has been sent to Anderson & Spence, Fountainhall Road.  Please let us know if you want to nominate a different pharmacy for your next prescription order.

New Patient Registration

handwriting_from_screenTo register with the Practice, please visit the Front Reception desk in person to obtain the necessary paperwork for completion.  Alternatively, you can print off forms from the links below in advance and bring them in to us. 

For adults and children over age 6, you will need to complete 2 forms for each person:

1) Registration Form (GPR)

2) New Patient Questionnaire - there are different forms for adults and children 

For children age 6 and under, you will need to complete 3 forms for each child:

1) Registration Form (GPR)

2) New Patient Questionnaire - Child

3) Immunisation Form

When you have completed the relevant forms, please bring these to the surgery, along with two forms of ID.  Adults should bring one document showing name and home address, eg utility bill.  Also, if you are on medication can you please take along the re-order part of your prescription if possible.

If you have a preferred Doctor, ie by name or gender, we will note this and do our best to respect your choice. However not all the Doctors in the Practice provide all services and specific Doctors may not be immediately available.

New patients may see our Healthcare Assistant for basic health screening, although this is not a requirement of registration with the Practice.
