CORONAVIRUS


If you have a new cough or high temperature or change in sense of taste or smell you must self-isolate and should arrange to be tested under the Scottish Government's `Test & Protect' programme.  Further advice is on the NHS Inform website at www.nhsinform.scot/campaigns/test-and-protect or phone 0800 028 2816.

If you have a cough or fever or change in sense of taste or smell and you need medical attention telephone dial 111 for NHS 24.  Patients in high risk groups should phone 111 as soon as they get symptoms.

PLEASE NOTE THAT OUR DOORS REMAIN LOCKED IN ORDER TO RESTRICT FACE-TO-FACE CONTACT WITH OUR HEALTH CARE TEAM.  Access to the building will be for pre-arranged appointments only for essential care.  Please bring your mobile phone with you.


On arrival at front door, a notice will advise you to call a mobile telephone number to let us know you have arrived.  You will be asked to wait in your car.  Tell us if you arrive on foot/by bus.  We will call you back to tell you when to enter the building.  If you do not have a mobile phone, please ring the doorbell and stand back from the door and wait.  Our Receptionist may have left the front desk to carry out a task - please be patient.


Adults to enter alone, unless carer required.  Children under age 16 may be accompanied by one carer.


Repeat prescriptions - our timescale for preparing prescriptions has been extended to 4 working days. All prescriptions will be sent to a nominated pharmacyAllow a further 2 working days before attending the pharmacy.  If you have not nominated a pharmacy your prescription has been sent to Anderson & Spence, Fountainhall Road.  Please let us know if you want to nominate a different pharmacy for your next prescription order.

New Patient Registration

handwriting_from_screenAs our doors are still closed due to the ongoing COVID-19 pandemic, we can accept registration paperwork via e-mail sent to hamilton.clinical@nhs.net or you can post completed forms through our letterbox.  You can print off the required forms from the links given below.  If you do not have access to a computer or printer, please call the Practice on Tel 01224 562888 to request that we send paper forms out by post.

Please note that during the pandemic we are discouraging patients who already have a GP in Aberdeen from changing Practices as this results in additional workload for both the Practice and Practitioner Services staff at a time when both services may be experiencing staff shortages.

Forms required to register:-

For adults and children over age 6, you will need to complete 2 forms for each person:

1) Registration Form (GPR)

2) New Patient Questionnaire - there are different forms for adults and children 

For children age 6 and under, you will need to complete 3 forms for each child:

1) Registration Form (GPR)

2) New Patient Questionnaire - Child

3) Immunisation Form

If you are on medication please provide us with the re-order part of your prescription, if possible.

If you have a preferred Doctor, ie by name or gender, we will note this and do our best to respect your choice. You should note, however, that not all the Doctors in the Practice provide all services and specific Doctors may not be immediately available.

New patients may see our Healthcare Assistant for basic health screening, although this is not a requirement of registration with the Practice.
