New Patient Registration


During the pandemic, the Scottish Government issued guidance, recommending patient moves between GP practices be paused, except where individuals/households had moved outwith practice boundaries or were new arrivals to the area. From 1 June 2022 this guidance will be withdrawn.

Unfortunately, we do not have the capacity currently to register new patients unless you are new to the area and do not have access to a GP practice or have moved house to an address that is out with your current practice’s catchment area.

We can however give you the details of the Primary Care team and they will either; put you on the waiting list for this practice or provide you with details of other practices in your surrounding area that do have the capacity to take on new patients.

As our doors are still closed due to the ongoing COVID-19 pandemic, we can accept registration paperwork via e-mail sent to or you can post completed forms through our letterbox.  You can print off the required forms from the links at the bottom of this page.  If you do not have access to a computer or printer, please call the Practice on Tel 01224 562888 to request that we send paper forms out by post.

Forms required to register:-

For adults and children over age 6, you will need to complete 2 forms for each person:

1) Registration Form (GPR)

2) New Patient Questionnaire - there are different forms for adults and children 

For children age 6 and under, you will need to complete 3 forms for each child:

1) Registration Form (GPR)

2) New Patient Questionnaire - Child

3) Immunisation Form

If you are on medication please provide us with the re-order part of your prescription, if possible.

If you have a preferred Doctor, ie by name or gender, we will note this and do our best to respect your choice. You should note, however, that not all the Doctors in the Practice provide all services and specific Doctors may not be immediately available.

New patients may see our Healthcare Assistant for basic health screening, although this is not a requirement of registration with the Practice.
